3. Property Characteristics
The year the house was originally built. A rough estimate is fine.
The approximate living area in square feet (not including the yard).
How many floors the home has above ground (e.g. 1 or 2).
What the outside walls are mostly made of.
What the roof is covered with. Asphalt Shingle is the most common.
The year the roof was put on or last replaced. Estimate if unsure.
What the home sits on. Leave blank if you're not sure.
Count a half-bath (toilet + sink, no shower) as 0.5 — e.g. 2.5.
'Attached' means the garage is connected to the house.
How the home is mainly heated.
If the wiring was ever upgraded, the year it was done.
If the pipes were ever upgraded, the year it was done.

Do you have working smoke detectors installed?
'Central Station' means an alarm monitored by a company 24/7.
Check this only if the home has indoor fire sprinklers.
Roughly how far the nearest fire hydrant is from the home.
Roughly how far the nearest fire station is.
5. Liability Exposures
Check if there is a pool on the property.
Check if there is a trampoline on the property.
Check if any dogs live at the home.
If you have a dog, its breed (some companies have breed rules).
6. Coverage Requested
How much it would cost to rebuild the home from scratch (not the market price).
How much your belongings inside the home are worth.
Protection if someone is injured at your home. $300,000 is typical.
The amount you pay out of pocket before insurance kicks in.
A separate deductible for wind or hail damage, if it applies.
7. Claims & History
How long you've lived at this home.
The company you currently have (or recently had) insurance with.
How long you've had continuous coverage with them.
How many insurance claims you've filed in the past 5 years (enter 0 if none).
